Admission/Referral
Our admission process has been designed to follow a seamless six-step process following initial contact or referral from GP, commissioner or Local authority. Before admitting a service user to our home, an assessment is completed to ensure the individual’s needs would be met and when necessary, to consider if additional resources were required such as staff training, recruitment or procurement of assisted technology. The Pre-admission assessment is undertaken by the registered manager at the prospective service user’s current place of residence. The RM asks about the person’s current situation, support needs and their short, medium and long-term ambitions. Sequel to the outcome of this assessment (i.e. if the assessment reveals that the individual falls within the scale of people that can benefit from our service), we then invite the prospective service user, carer/family members or anybody with care responsibilities to make an informal visit to inspect our setting.

This offers the opportunity to look around the facility and ask our team any questions they might have. If the service user and relatives are happy with our facility, we then proceed to arrange for the individual to return for a day visit when there will be ample time and further opportunity to meet with other service users, and staff, and ask questions to help them make informed decisions about the services on offer. If there is an agreement by all parties that a placement at our residence is in the best interests of the individual, the registered manager then confirms this in writing to the service user and/or the responsible individual. This writing will involve a support and funding proposal for the local council for approval. We then await funding confirmation from the local authority or provider before placement is completed. Once funding is confirmed we can set a move-in date and the RM will be on hand to support the person through the transition process to make the move as seamless as possible.
